What ACCESS is

ACCESS is a new program from the Medicare Innovation Center.

Medicare pays participating care organizations for helping you improve or keep control of your condition, not for the number of visits.

Your Medicare benefits, coverage and rights do not change.

You can keep seeing any Medicare provider you choose.

How joining works

    Pick your track above and start a chat.

  2. 2

    Create your account with your phone number and share your Medicare card details.

  3. 3

    We check your eligibility with Medicare. This usually takes about a minute.

  4. 4

    If eligible, you review the program terms and decide whether to join. Enrollment is voluntary and joining is always your choice.

  5. 5

    You meet a Curai clinician on a video visit. They confirm your diagnosis, review your history and take your starting measurements. If you have lab results from the past year, have them ready to upload during the visit.

    Once your clinician confirms, you choose whether to enroll. Enrollment is voluntary. If you join, your care team stays with you for the care period and sends progress updates to your own doctor if you want them to.

Not everyone who applies can enroll. Enrollment is voluntary and always your choice.

Medicare places some applicants at random into a comparison group. If that happens, you cannot join for 12 months. It is not about you or your health, and your Medicare does not change.

You can also be ineligible if you have Medicare Advantage or are already enrolled with another ACCESS organization in the same track.

Whatever the outcome, Curai can still help with everyday health questions.

Important information: The ACCESS Model is voluntary and is available only to eligible people with Original Medicare who have a qualifying condition. Curai Health will verify your eligibility with Medicare before you enroll. A small number of eligible applicants may be randomly assigned by Medicare to a comparison group and will not be able to enroll in the same ACCESS track for 12 months. To provide your care and support Medicare's administration and evaluation of the model, Curai Health and CMS may collect, use and share relevant health, enrollment and Medicare claims information as permitted by law. Your Medicare benefits, coverage and rights do not change, and you may continue to see any Medicare provider you choose.

Your doctor stays your doctor

ACCESS is built to work alongside your primary care doctor, not replace them.

Your doctor can refer you. With your permission, your Curai care team sends them regular updates on your progress.

If your doctor referred you, we will ask for their name and practice so we can keep them informed.
